چکیده
A 47-year-old man presented with 1-day history of dyspnoea and near syncope. Electrocardiography showed sinus rhythm and chest X-ray showed mild cardiomegaly. Serum troponin I was mildly elevated at 0.371 ug/l. Arterial blood gases showed hypoxaemia with pO2 of 55mmHg. Transthoracic echocardiography revealed dilated left atrium (LA) and severe mitral regurgitation refluxing into pulmonary veins (Figure 1B). The mitral regurgitation was secondary to a possible perforation in the prolapsed anterior mitral valve leaflet (AMVL; Figure 1A). There was a mobile echodensity across the interatrial septum (IAS; Figure 1D). Computed tomography of the pulmonary arteries revealed the presence of filling defects in branches of both the right and left pulmonary arteries, consistent with pulmonary embolism. The patient was anti-coagulated. A pre-operative transoesophageal

Motion of the interatrial septum in acute mitral regurgitation. Clinical and experimental echocardiographic studies.
The interatrial septal echocardiograms from 15 patients with acute mitral regurgitation due to ruptured chordae tendineae were compared with those from 14 normal subjects. On the cross-sectional echocardiogram, the interatrial septal configuration in patients with chordal rupture showed a characteristic pattern in which the interatrial septum (IAS) was flat or slightly convex toward the left at...
متن کاملPercutaneous septal sinus shortening: a novel procedure for the treatment of functional mitral regurgitation.
BACKGROUND The septal-to-lateral (SL) mitral annular diameter is increased in functional mitral regurgitation (MR). We describe a novel percutaneous technique (the percutaneous septal sinus shortening system) that ameliorates functional MR in an ovine model. METHODS AND RESULTS Sheep underwent rapid right ventricular pacing to obtain moderate to severe functional MR with SL enlargement. The p...
متن کاملRelation of interatrial duration and p wave terminal force as a novel indicator of severe mitral regurgitation.
OBJECTIVES Interatrial duration is defined as prolonged p wave on electrocardiogram. p waves with a negative terminal phase recorded in V1 enclosing an area of one small square on the electrocardiogram is significantly and strongly correlated with interatrial duration. The aim of study was to investigate whether interatrial duration with p terminal force can be used as reflection of echocardiog...
متن کاملMitral Valve Disease Complicated by Left-to-right Shunt at Atrial Level.
THE association of mitral stenosis with atrial septal defect (Lutembacher's syndrome 1) is well known but, when strict criteria of definition are applied, it is rare. For example, six, one, and no cases, respectively, were found in series of 2,000,` 500,4 and 400) patients undergoing commissurotomy for mi-tral stenosis, and only two cases exist in the pathologic files of the Mayo Clinic." Green...
متن کاملReal-time cross-sectional echocardiographic evaluation of the interatrial septum by right atrium-interatrial septum-left atrium direction of ultrasound beam.
Real-time cross-sectional echocardiography was performed to record the interatrial septal echogram by right atrium-interatrial septum-left atrium (ASA) direction of the ultrasound beam by positioning the transducer to the right of the sternum. The configuration of the interatrial septum (IAS) and the change of the configuration through each cardiac cycle were studied in 10 normal subjects and 2...
